G. Kevin Gifford, DC, EdD is an in-network provider for Medicare and many other insurance companies, and the Arizona Department of Education’s Empowerment Scholarship Account (ESA).

Upon qualification, ESA pays for examination and treatment of children with learning-based disabilities. Dr. Gifford is also in-network with the State of Arizona’s Medicaid program called Arizona Health Care Cost Containment System (AHCCCS), and some 3rd – party AHCCCS insurers – not including Mercy Care.

We are happy to bill your insurance companies for the services they pay for. Included services commonly are examination of the spine for subluxations, and chiropractic manipulation of the spine.
Services not included in insurance reimbursement:

1. Neurological examinations that diagnose the central integrative state of the brain.

2. Neuropsychological examinations of:

a. ADD/ADHD

b. Autism spectrum disorder

c. Behavioral disabilities

d. Dementia

e. Dyslexia

f. Emotional disabilities

g. Genetic disorders such as Down Syndrome and genetic deletions

h. Intelligence

i. Intellectual Impairment

j. Learning disorders

k. Sensory processing disorder

l. Other neuropsychological and cognitive conditions.

3. Therapy to treat these conditions.

Your responsibility is to pay for what you want us to do. We help by accepting all major credit cards, including PayPal and Venmo. Care Credit is often a good option if you want 0% interest. Care Credit offers 0% interest if you comply strictly with their repayment schedule. Cash and checks are welcome if you’d like.
